Your child's pediatrician or physician will need to make a referral for therapy. The referral/intake form can be downloaded and taken to your child's physician for the therapy order to "evaluate and treat". There is a place for the physician to sign and date.
When you schedule your child's initial evaluation, we can mail the intake packet to you or you can download the forms by clicking on the links. The intake forms will need to be completed prior to the evaluation time so the scheduled time can be devoted to evaluating your child.
When your child is referred for therapy at made2move: therapy4kids, there is a verification of benefits that is required. The Insurance benefits worksheet helps you know which information we will be needing to obtain funding authorization in a timely manner.
If your child has been receiving therapy from another agency, the transfer of services form will need to be completed before our facility can verify benefits.
